Medical and Health Forms

Getting ready for a healthy new school year at Carolina Day.

Each year, Carolina Day School families are required to enter and/or update important medical records and forms for their students. CDS uses Magnus Health to manage our student health records. Starting in July, families will begin to receive reminder emails from Magnus Health notifying them that they must take action with their student's online Student Medical Record (SMR). The emails will list any incomplete requirement(s) that will need to be updated. Emails are sent on a weekly basis so if families have an incomplete requirement, they will receive reminder emails throughout the summer to ensure health information compliance. 

Families can access Magnus Health securely from within the My CDS Resource Boards.

Medical and Health Forms Deadline: August 1

Within Magnus Health, items requiring parental action are marked with a “To Do” flag. Some items will have saved information from last year that simply needs to be approved. Other items that have expired will need to be signed or resubmitted for the upcoming school year. 

For returning families, all medical forms (apart from physicals & immunization records) expired on July 1. Therefore if your student will need any of the following it will need to be updated and submitted by August 1st.

Athletic Forms

As an NCISAA and CDS requirement for athletic participation, families of middle and upper school student-athletes must complete the following forms prior to participating in preseason activity. 

  • 2026-2027 NCISAA Pre-Participation Physical Exam Form Any child that wishes to participate in the CDS athletic program must have an updated yearly pre-participation physical in their Magnus Health Portal. Students will not be allowed to participate in any CDS practice/match/game without this document. The form must be filled out by a physician, either the student’s pediatrician or at any local urgent care or minute clinic that completes Sports Physicals. (This form does not take the place of your child’s annual physical or wellness check.) Completed forms must be uploaded to the Magnus Health portal. (We cannot accept paper documentation.)

The rest of the forms are also required and will be collected by coaches at the first practice. Coaches will have extra copies available, and additional forms will also be available through the Athletics Office. As the NCISAA increases its requirements for athletic documentation, this process will help us maintain accurate records and ensure all required paperwork is completed and accounted for.

  • PT Solutions Consent to Treat form Carolina Day School partners with PT Solutions to provide a certified athletic trainer at all games and practices. Any student-athlete who will be participating in after school sports will need a signed consent form on file to be treated by our Sports Medicine team. 
  • Sudden Cardiac Death Information and Acknowledgement Form This form provides information for the student and parents on the rare occurrence of sudden cardiac death in youth athletes. All student-athletes and their parents must review, sign, and upload it to the Magnus Health Portal.
  • NCISAA Consent to Participate and Medical Release Form The NCISAA consent to participate and Medical Release Form is required prior to participating in athletics for Carolina Day.
  • Gfeller-Waller Concussion Awareness Act Form To help ensure the safety of our student-athletes, we have implemented a concussion management protocol following the Gfeller-Waller Concussion Awareness Act. All student-athletes and their parents must review the Concussion Awareness Act form, sign it, and upload it to the Magnus Health Portal. 
    • Additionally: All students who are participating in an impact sport need to establish a yearly concussion baseline prior to their first game of the season.
