Medical and Health Forms

Getting ready for a healthy new school year at Carolina Day.

Each year, Carolina Day School families are required to enter and/or update important medical records and forms for their students. CDS uses Magnus Health to manage our student health records. Starting in July, families will begin to receive reminder emails from Magnus Health notifying them that they must take action with their student's online Student Medical Record (SMR). The emails will list any incomplete requirement(s) that will need to be updated. Emails are sent on a weekly basis so if families have an incomplete requirement, they will receive reminder emails throughout the summer to ensure health information compliance. 

Families can access Magnus Health securely from within the My CDS Resource Boards.

 

Medical and Health Forms Deadline: August 1, 2024

All medical and health forms are due before students arrive back on campus and must be submitted by August 1st. This deadline provides our Nurses Office with time to review forms and follow-up with families with questions or concerns prior to the start of school. 

Once in Magnus you will find items marked with a “To Do” flag that require parental action. Some forms can be completed completely online and some will require printing and submitting via the Magnus Portal. Additionally, if your student will need any of the following it will need to be submitted prior to the start of school. (These forms are available in Magnus.)

• Medication Authorization Form 
• Asthma Action Plan 
• Allergy Action Plan 
• Medication Self Carry Form 

Also, if your child is rising Kindergarten, rising Grade 7, or rising Grade 12 they will need a copy of their updated immunizations for the upcoming school year. New students and rising K students will need a current physical submitted as well.

Note for returning families: Some items will have saved information from last year that simply needs to be approved. Items that have expired will need to be signed or resubmitted for the upcoming school year. (All medical forms, apart from physicals & immunization records, expire on July 1 each year and therefore will need to be updated and submitted prior to the start of the new school year).

Athletic Forms
