Athletic Requirements and Health Forms
NCISAA and CDS Required Athletic Forms:
Prospective student athletes and families can prepare for the upcoming year early by getting the upcoming year's Pre-Participation Physical Exam Form (sports physical) completed over the summer and submitting it electronically in July to the Magnus Health Portal for approval before August 1. A link to the Magnus Health Portal can be found in My CDS.
Athletic Forms
As an NCISAA and CDS requirement for athletic participation, families of middle and upper school student-athletes must complete the following forms prior to participating in preseason activity.
- 2026-2027 NCISAA Pre-Participation Physical Exam Form Any child that wishes to participate in the CDS athletic program must have an updated yearly pre-participation physical in their Magnus Health Portal. Students will not be allowed to participate in any CDS practice/match/game without this document. The form must be filled out by a physician, either the student’s pediatrician or at any local urgent care or minute clinic that completes Sports Physicals. (This form does not take the place of your child’s annual physical or wellness check.) Completed forms must be uploaded to the Magnus Health portal. (We cannot accept paper documentation.)
The rest of the forms are also required and will be collected by coaches at the first practice. Coaches will have extra copies available, and additional forms will also be available through the Athletics Office. As the NCISAA increases its requirements for athletic documentation, this process will help us maintain accurate records and ensure all required paperwork is completed and accounted for.
- PT Solutions Consent to Treat form Carolina Day School partners with PT Solutions to provide a certified athletic trainer at all games and practices. Any student-athlete who will be participating in after school sports will need a signed consent form on file to be treated by our Sports Medicine team.
- Sudden Cardiac Death Information and Acknowledgement Form This form provides information for the student and parents on the rare occurrence of sudden cardiac death in youth athletes. All student-athletes and their parents must review, sign, and upload it to the Magnus Health Portal.
- NCISAA Consent to Participate and Medical Release Form The NCISAA consent to participate and Medical Release Form is required prior to participating in athletics for Carolina Day.
- Gfeller-Waller Concussion Awareness Act Form To help ensure the safety of our student-athletes, we have implemented a concussion management protocol following the Gfeller-Waller Concussion Awareness Act. All student-athletes and their parents must review the Concussion Awareness Act form, sign it, and upload it to the Magnus Health Portal.
- Additionally: All students who are participating in an impact sport need to establish a yearly concussion baseline prior to their first game of the season. To ensure your child is eligible to participate in his or her first game of the season, instructions will be provided for parents to guide them through SWAY baseline concussion testing at home.
Concussion Management:
In 2021, Carolina Day adopted a new form of Concussion testing called Sway Medical. All student-athletes must undergo a yearly cognitive baseline assessment prior to the start of any contact sport.
Sway is a user-friendly, mobile device-based testing program specifically designed for the management of sports-related concussions. The Sway System is a research-backed assessment tool that includes a battery of balance tests, cognitive tests, symptom tracking, and on-field assessments. The system is a clinically validated objective assessment for baseline testing, sideline assessment, and post-injury evaluation.
To increase and ensure the safety of our student-athletes, Carolina Day follows the NC Gfeller-Waller Concussion Law, which has three main areas of focus: education, emergency action and postconcussion protocol implementation, and clearance/return to play or practice following concussion.
CDS also has in place a concussion management team (which consists of our school nurse, Athletic Trainer, coaches, student advisors, and team doctor) that ensures a smooth and consistent return-to-play process after a concussion has been diagnosed. Once a concussion has been diagnosed, the concussion management team ensures that the athlete completes both phases of the GFW concussion protocol, which includes a return-to-learn phase followed by a gradual 5-step return-to-play phase.

Sports Medicine
Our certified athletic trainer, Nikolaus Holmberg, along with our strong partnerships with EmergeOrtho/Blue Ridge Bone and Joint and PT Solutions allows us to meet the needs of our large and active athletic community. CDS student-athletes will have access to our athletic training rooms in the Nash Athletic Center and Sgro Athletic Complex.
- The safety of your child is paramount. If your child has an existing medical condition that you believe needs to be discussed, update their Magnus Health Portal and contact Nikolaus Holmberg via email directly (athletictrainer@carolinaday.org) as needed.
- If an injury occurs and your child misses practice and or a competitive match, he/she will not be able to return to practice or competition without an assessment from our athletic training staff or written permission from your child’s physician.
- Stay hydrated! Please have your student-athlete bring a labeled water bottle on a daily basis to practices and games.
